Dr. Lee is a Certified DOT Medical Examiner.  For your DOT Medical Card, you can come in any time during our regular office hours - Monday to Friday 8:30AM to 4:30PM.  You never need an appointment to be seen.  Bring any forms and related medical reports that you have.  We will have to check a urine sample as part of your exam. 

After successful completion of your exam, Dr. Lee will give you a valid Medical Card to bring to the Registry of Motor Vehicles to get your CDL or Hoisting License.

Drug Testing

Dr. Paul Lee is currently one of 2 physicians on Cape Cod certified as MRO (Medical Review Officer), licensed to conduct and certify drug test results for the Coast Guard License Exam and CDL drivers for the DOT (Department of Transportation).  Dr. Lee has available the NIDA-5 drug panel from SAMSHA approved labs.  Other drug panels and Breath Alcohol Tests are also available.  Our office is a collection site for many companies with CDL drivers and marine employees.


USCIS Immigration Medical Exams

Dr. Lee is a certified Designated Civil Surgeon, the only physician in the Upper Cape licensed by the United States Citizenship and Immigration Services (USCIS) to perform your immigration exam, update your Vaccines requirement, and complete your medical form.  Prior to your visit, you should go online to the web site: U.S. Citizenship and Immigration Service. Download and print the I-693 form, which is a 9 page document.  Complete all the personal information.  For your visit, bring your I-693 form plus any medical records, especially past vaccine and immunization records.  We will thoroughly check all your records.  We will do any necessary testing for TB, as well as drawing your blood to test for syphilis.  We will administer any required vaccines in compliance with the USCIS guidelines.  We will then approve and certify your I-693 form.  You will need to return in 2-3 days to pick up your completed I-693 form and all supporting documents to send to the USCIS.
